      • SPORTS PHYSICALS
         
        A sports physical is required for all students intending to play sports at Akron Central School during the next school year.  The sports physical is good for one (1) calendar year, which includes all three ACS sports seasons: fall, winter and spring.
         
        A student must use their own private physician, nurse practitioner or physician's assistant to conduct the physical.  NYS requires the doctor to complete the new NYS Required School Health Examination Form, available HERE.

        Sports Physicals can be returned via fax at (716)542-5057 or mailed to: Health Office, Akron Schools, 47 Bloomingdale Avenue, Akron, NY 14001.
